PLEASE NOTE: When uploading labs and reports - please limit no more than 8 documents prior to a visit!  Scan all your labs and send as one file, please don't scan one page at time and upload each page, this slows down the system.  Ideally scan your labs into several files and then upload or have your MD fax your labs to my office at 941-203-1014.  Thank you!
